- A completed and signed application form.
- A letter to the Ambassador of the Republic of Indonesia clarifying the purpose, amount and the recipient of gift/donation, as well as other relevant information.
- A gift certificate explaining that the donation/gift would be granted to an individual or an agency/institution in Indonesia.
- Invoice and packing list of the goods. At the end of the invoice/packing list, please insert this statement “no commercial value and value for custom purpose only”.
- Enclose correspondences between donor and recipient.
- Letter of approval from relevant authorities in Indonesia.
